Surgery Straightens the Eye — Therapy Fixes the Cause

Surgery can align the eye cosmetically — but it cannot teach the brain and eye to work together. Vision therapy addresses the underlying binocular dysfunction that causes squint, reduces recurrence risk, and builds the depth perception that surgery alone cannot deliver.

What Surgery Does — and Doesn't Do for Squint

Strabismus surgery repositions the eye muscles to achieve cosmetic alignment. It is an appropriate intervention for certain types and angles of squint. However, surgery does not retrain the brain to fuse the images from both eyes — and does not address the suppression that causes many squints to recur after surgery.

Vision therapy is the evidence-based rehabilitation that addresses the neurological component of strabismus. For accommodative esotropia in particular, vision therapy combined with appropriate spectacle correction can resolve the squint entirely without surgery. For constant large-angle squints, surgery followed by vision therapy produces significantly better binocular outcomes than surgery alone.

Types of Squint Where Vision Therapy Is Most Effective

01 Accommodative esotropia — inward eye turn triggered by focusing effort. Corrective lenses reduce the turn; vision therapy builds fusion to eliminate it. Surgery is rarely needed in pure accommodative cases
02 Intermittent exotropia — an outward drift that appears during fatigue or daydreaming. Vision therapy is the preferred first-line treatment for intermittent cases before surgery is considered
03 Post-surgical residual squint — some alignment remains after surgery, or the squint recurs. Vision therapy addresses the binocular fusion deficit that surgical correction cannot fix
04 Strabismic amblyopia — squint-caused lazy eye. We treat both conditions simultaneously — alignment therapy and binocular vision rehabilitation working together
05 Pre-surgical preparation — improving convergence and fusion reserves before surgery improves surgical outcomes and reduces the likelihood of recurrence post-operatively

Comprehensive Strabismus Evaluation

Anti-Suppression Therapy

We break the brain's learned suppression of the deviating eye using dichoptic activities, Brock string, red-green anaglyphs, and prism-based vergence training. This is the foundational step before fusion work can begin.

Fusional Vergence Training

Both convergence and divergence ranges are expanded to give the brain a stable fusion zone. This reduces the frequency and angle of the squint and builds genuine binocular function — not just cosmetic alignment.

Stereopsis Development

Once fusion is stable, we develop fine stereopsis (3D depth perception) through graduated stereoscopic activities. This is the outcome that differentiates vision therapy from cosmetic surgical alignment alone.

Can vision therapy cure squint without surgery?
For certain types — particularly accommodative esotropia and intermittent exotropia — vision therapy can eliminate or substantially reduce the squint without surgery. For constant large-angle squints, surgery may be required first. We will advise clearly after your evaluation about what is achievable without surgery in your specific case.
My child had squint surgery two years ago. Do they still need vision therapy?
Post-surgical vision therapy is often recommended to build the binocular fusion and stereopsis that surgery alone cannot produce. If your child still has suppression, poor depth perception, or reading difficulties — or if the squint is showing early signs of recurrence — a functional binocular vision evaluation is strongly advisable.
How do I know if my child's squint is intermittent or constant?
Parents often notice the eye drifting when the child is tired, ill, or daydreaming — but not all the time. This intermittent pattern is a critical diagnostic finding. Bring any videos or photographs showing the squint to your evaluation — these help us classify severity and frequency accurately.
What age can squint vision therapy start?
Formal in-clinic vision therapy is typically suitable from age 5–6 when a child can follow instructions reliably. For younger children, parent-guided activities are provided with regular clinic reviews. There is no upper age limit — adult strabismus therapy is offered and produces meaningful functional improvements.

Many adults want more than cosmetic alignment — they want to drive safely, judge distances accurately, and read without effort. Adults who have lived with strabismus since childhood, or who develop an acquired squint after neurological injury, can benefit significantly from vision therapy.
