How to Tell the Difference

Many children diagnosed with ADHD have an underlying, undetected functional vision problem - particularly convergence insufficiency, poor eye tracking, or visual processing deficits - that produces inattention, avoidance of reading, and classroom restlessness that closely mimics ADHD. Research confirms that children with convergence insufficiency score significantly higher on validated ADHD symptom rating scales than children with normal vision, even when no ADHD is present; a comprehensive binocular vision evaluation is therefore recommended for any child showing ADHD-like symptoms before or alongside a psychiatric assessment. Clinical evidence: Granet et al. (Strabismus, 2005) found convergence insufficiency was 3× more prevalent in children with ADHD than in matched controls. Borsting et al. (Optometry and Vision Science, 2012) demonstrated that treating CI with office-based vision therapy produced significant reductions in parent-reported ADHD symptom scores alongside improved reading performance - with improvements strongly correlated with vergence gains. Up to 1 in 6 children referred for ADHD evaluation have been found to have an underlying, untreated convergence insufficiency.

Why Are Vision Problems Mistaken for ADHD?

A child with an undetected convergence insufficiency or visual processing problem will avoid near tasks, lose their place while reading, become restless during homework, and appear inattentive or disruptive in class. These are identical to the behavioural hallmarks of ADHD.

Unlike ADHD, vision-related attention difficulties are specifically triggered by near visual demands - reading, writing, and screen tasks - and improve when visual tasks are removed. Standard ADHD assessments do not include comprehensive binocular vision testing, so these conditions are routinely missed. Research shows that children with convergence insufficiency score significantly higher on ADHD symptom rating scales than children with normal vision - even when they do not have ADHD.

This does not mean ADHD is not real or that all ADHD is a vision problem. However, a comprehensive functional vision evaluation before or alongside an ADHD assessment ensures that treatable vision conditions are not overlooked - and that children receive the right treatment.

Which Vision Conditions Mimic ADHD Symptoms?

These conditions are missed by standard school vision screenings and routine eye tests.

Convergence Insufficiency

The eyes struggle to converge for near tasks. Causes double vision, headaches, reading avoidance, and loss of place. Produces ADHD-identical symptoms on standardised rating scales. Highly treatable with office-based convergence therapy.

Visual Processing Disorders

Deficits in visual memory, visual discrimination, visual-spatial processing, or visual-motor integration. The child may see clearly but cannot efficiently process and retain what they see - directly impairing reading comprehension, maths, and writing skills.

Accommodative Dysfunction

The eyes cannot sustain clear focus during near work. The child gets blurry vision within minutes of reading, triggering headaches and rapid work avoidance. Often missed entirely in standard tests which only measure static visual acuity.

Frequently Asked Questions

Can vision therapy replace ADHD medication?
Vision therapy does not treat ADHD itself. However, if a child's attention difficulties are caused primarily or partly by an undetected vision problem such as convergence insufficiency, treating that vision problem can dramatically reduce attention difficulties without medication. If a child has both ADHD and a vision problem, both require separate appropriate treatment.
My child passed the school eye test - could they still have a vision problem?
Yes - absolutely. School vision screenings only test distance visual acuity (the standard eye chart). They do not assess binocular vision, convergence, accommodation, eye tracking, or visual processing. All the conditions that cause ADHD-like symptoms are missed entirely by standard screenings.
At what age can children be evaluated?
