What Your MRI Does Not Show

Standard MRI at 1.5T or 3T resolution cannot visualise the micro-scale white matter tracts disrupted by diffuse axonal injury. These tracts carry visual pathway signals. All of the following TBI visual problems can be present with a completely normal MRI:

  • Convergence insufficiency and near-vision fatigue
  • Saccadic disruption causing reading difficulty and dizziness
  • Photophobia from cortical hyperexcitability
  • Vestibular-visual mismatch causing motion sensitivity and balance problems

What visual problems are most common after TBI?

The most common visual problems following traumatic brain injury are convergence insufficiency (difficulty maintaining eye alignment at reading distance), oculomotor disruption (poor pursuit and saccadic control), photophobia (light sensitivity), diplopia (double vision), and visual field defects. Many TBI patients also experience visual processing slowness and difficulty with figure-ground and spatial tasks. Several of these problems co-occur in the same patient and each requires its own rehabilitation approach.

My MRI was clear but I still have visual problems. Is this possible?

Yes, and this is common after TBI. Standard MRI resolution is insufficient to visualise the small-scale white matter tracts and visual pathway connections that are disrupted by diffuse axonal injury. Functional visual pathway disruption is invisible to standard neuroimaging, including 1.5T and 3T MRI. The visual problems are real: they simply require a functional vision assessment to identify, not an imaging study. Our Anna Nagar evaluation assesses these directly.

How long after TBI should I wait before starting vision rehabilitation?

There is no mandatory waiting period. As soon as you are medically stable, safe to travel, and able to sustain a 60 to 90 minute evaluation, you can attend our Anna Nagar clinic. Early intervention takes advantage of the neuroplasticity that is greatest in the months immediately following injury. For patients in the acute rehabilitation phase, we can co-ordinate with the treating team to time the initial evaluation appropriately.

Can photophobia after TBI be treated?

Yes. Post-TBI photophobia is commonly caused by cortical hyperexcitability in the visual cortex. Precision-tinted lenses at specific wavelengths reduce the cortical overload driving the photosensitivity. This is a therapeutic intervention: the tint must be identified through a standardised assessment process, not selected by preference. We assess for this at our Anna Nagar clinic as part of the full TBI visual evaluation.

Can I attend telehealth sessions from home after the initial evaluation?

Yes. After the initial in-clinic evaluation and the first few in-person sessions at our Anna Nagar East clinic, follow-up reviews and some home programme sessions can be conducted via secure telehealth video call. This is particularly useful for TBI patients who find travel fatiguing or who live outside North Chennai. In-clinic sessions are needed for any procedures requiring specialist equipment.
