When Surgery Is Not the Right First Step for Squint
Strabismus surgery repositions the extraocular muscles to improve eye alignment. It does not teach the brain to use both eyes together. Without binocular fusion, the eyes frequently drift back to their previous misaligned position after surgery - a well-documented phenomenon called surgical recurrence.
For accommodative squint (the most common type in children 2-6 years), optical correction alone often controls the deviation. Vision therapy builds the fusional vergence reserves that allow the eyes to maintain alignment under visual stress. For non-accommodative strabismus, vision therapy before surgery builds sensory readiness and after surgery maintains alignment - significantly reducing recurrence risk.

Non-accommodative sensory strabismus may require 12 months of therapy. Prognosis is discussed clearly after the initial evaluation.
Can vision therapy prevent squint surgery?
For accommodative squint - yes, in many cases. Full hyperopic correction controls the squint entirely in a significant proportion of cases, and vision therapy maintains alignment by building fusional reserves. For large-angle non-accommodative strabismus, surgery is often necessary for cosmetic alignment, but vision therapy before and after surgery improves binocular outcomes and reduces recurrence risk.
Does vision therapy work for adults with squint?
Yes, particularly for convergence insufficiency, decompensated exophoria, and post-concussion strabismus. Adults with functional binocular vision conditions respond well to structured vergence therapy. Those with long-standing manifest strabismus from childhood have more limited binocular potential but may still achieve improved alignment and reduced diplopia.
